Location: St Francis

Position is located in beautiful Sauk City, Wisconsin.

Operating Hours are 8AM-4:30PM, Monday-Friday.

Responsibilities
  • Answering medication related questions from residents, residents’ families and nursing staff.
  • Attending Admission/Discharge Meetings on a monthly basis if scheduled that same day.
  • Attending Interdisciplinary Meetings on a weekly basis if scheduled that same day.
  • Attending Quality Assurance Meetings quarterly if scheduled that same day.
  • Checking prescriptions for accuracy.
  • Completing at least 50% of monthly medication reviews each month, unless staffing is less than 50% of days (to be determined at time of hire).
  • Completing new medication reviews prior to dispensing.
  • Completing Change in Condition medication reviews as necessary.
  • Contacting insurance companies as needed.
  • Contacting prescriber for clarification/recommendations as needed.
  • Discharge medication counseling.
  • Attain functional knowledge of ECS/American Data eMAR software.
  • Attain mastery of QS1/Prime Care/Red Sail Pharmacy Software.
  • Triaging tasks and directing staff to keep monthly cycle fills completed in a timely manner.
  • Taking on‑call 50% of weekdays and weekends, rotating holidays and holiday weekends annually.
  • Willingness to come in on non‑staffing day for monthly pharmacy meetings six times a year.
Preferred Qualifications
  • Experience billing and running revenue reports.
  • Experience in a long‑term care pharmacy setting.
  • Experience conducting monthly medication reviews.
  • Familiarity with sections pertaining to pharmacy services in CMS’s most recent State Operations Manual Appendix PP.
  • Familiarity with applicable state rules and regulations pertaining to long‑term care pharmacy practice.
  • Credential of a Certified Geriatric Pharmacist or equivalent.
  • Willingness to assist in formulating policies and procedures as needs arise.
