Family Services – Admission & Case Coordinator

Position Overview

The Family Services – Admission & Case Coordinator plays an important role in helping families navigate their first steps into care and ensuring a smooth, supportive experience throughout treatment.

As a member of the Family Services Team
, this position serves as an important connection between families, admissions, and the treatment team. The role combines admissions coordination, family communication, case support, and continuity of care.

We are looking for someone who is organized, responsive, compassionate, and able to confidently manage competing priorities while creating an exceptional experience for clients and families.

Key Responsibilities
  • Review incoming referrals and coordinate the admission process from initial inquiry through arrival.
  • Serve as a consistent point of communication and support for families throughout the admission and treatment process.
  • Communicate with families, referral sources, providers, and internal departments to ensure timely and appropriate admissions.
  • Help families understand what to expect before, during, and after admission.
  • Coordinate case-related needs throughout treatment and support continuity of care.
  • Assist with discharge planning, follow-up coordination, and communication with outside providers as appropriate.
  • Maintain accurate and timely documentation and patient records.
  • Ensure admission and case coordination processes meet organizational and regulatory requirements.
  • Collaborate closely with clinical, medical, nursing, utilization review, and operations teams.
  • Identify barriers or delays and proactively work toward solutions.
  • Participate in treatment and team meetings as appropriate.
  • Help continuously improve the client and family experience.
  • Share responsibility for admissions coverage through a planned rotating on-call schedule.
Rotating On-Call Schedule

Because families may need access to care outside traditional business hours, members of the Family Services Team participate in a shared, rotating admissions on-call schedule
.

On-call responsibility is intentionally built into the position's overall compensation and scheduling structure rather than treated as an unexpected additional responsibility.

To support work-life balance:

  • On-call weeks are scheduled and rotated in advance among qualified team members.
  • The regular work schedule includes planned days off and flexibility to help balance on-call responsibilities.
  • Team members are not expected to be continuously onsite while on call; the role primarily requires appropriate availability and responsiveness when an admissions need arises.
  • Leadership works to distribute on-call responsibility equitably and protect scheduled time off whenever possible.
  • Clear escalation processes and leadership support are available when additional assistance is needed.

Qualifications
  • Bachelor's degree in healthcare administration, behavioral health, social services, business, or a related field preferred.
  • Experience in healthcare coordination, admissions, behavioral health, case management, customer service, or a related role preferred.
  • Strong interpersonal and relationship-building skills.
  • Excellent written and verbal communication.
  • Strong organization and time-management abilities.
  • Ability to prioritize and make sound decisions in a fast-moving environment.
  • Comfortable working independently while remaining highly collaborative.
  • Knowledge of healthcare documentation, confidentiality, and regulatory requirements preferred.
  • Ability to remain composed and professional when supporting families experiencing stressful or urgent situations.
