Looking for a dynamic Therapist who will provide direct therapy services for clients including individual, group and family therapy, treatment planning, case management and related functions including coordinating follow up treatment of clients. Vive is a Joint Commission Accredited Facility.
Qualifications- Education:
Minimum of a Master’s Degree in clinical social work, marriage and family therapy, or licensed professional counselor. - Training and
Experience:
Intervention techniques of individual, group and family therapy in working with clients and their families. A basic knowledge of educational, psychological, family and social work systems as related to special education and behavior treatment. - Must have a working knowledge of treatment plan development and implementation including charting and progress notes and related documentation skills.
- License/Certification:
Licensed Clinical Social Worker, MSW (Preferred)
- Oversee and facilitate the overall therapeutic care of each student on caseload
- Conduct a minimum of 1 Individual therapy session with each student
- Ensure that you run assigned group therapy
- Maintain weekly correspondence with parents of assigned students
- Participate in the planning and implementation of groups
- Be available for weekend on-call work rotation
- Create and maintain required charting on students
- Create an initial treatment plan within 24 hours of admittance for clients on caseload
- Create and maintain an individualized treatment plan for each client on caseload within 7 days from admission
- Complete discharge plan for students within 7 days from clients discharge date
- Connect students with transitional care as part of discharge plan
- Disseminate student information to staff as needed to facilitate treatment
- Participate in weekly Clinical Team Meetings
- Provide documentation and updates to third parties (insurance, doctors, therapists, etc.), adhering to standards of confidentiality
- Follow Expectations and Protocols as outlined in the Joint Commission manual
- All other tasks assigned by Clinical Director
VIVE Adolescent Care is a inpatient acute stabilization facility. At Vive our mission is to provide "in Crisis" youth with short term, innovative, individualized medical and clinical treatment while providing safety, stabilization, and renewed hope for patients and their families.
