Admission and Case Coordinator

The Admission and Case Coordinator is responsible for managing the admission process for new patients and coordinating the ongoing care of existing patients. This includes:

• Reviewing patient referrals and determining eligibility for admission
• Coordinating with healthcare providers to ensure appropriate care plans are in place
• Communicating with patients and their families to provide information and support
• Managing patient records and ensuring compliance with regulatory requirements
• Collaborating with other members of the healthcare team to ensure high-quality care

• Bachelor's degree in healthcare administration or related field
• Experience in healthcare coordination or case management
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of healthcare regulations and compliance requirements
