Job Summary
CNA and PCA provide non‑medical service and assistance to clients in their homes and communities who, due to advanced age or physical or mental disability, cannot perform the required services. The caregiver's main objective is client satisfaction and safety, both for themselves and for the client. Caregivers must be sensitive to the needs of others, maintain a professional manner, and display a positive attitude.
ESSENTIALFUNCTIONS
The CNA/PCA provides service to a client based on and limited to the client’s care plan developed by the RN, the client, and/or the client’s family.
Services are tasks that are necessitated by the client’s disability or chronic condition that limits the ability to perform functional activities or daily living (ADL’s) and instrumental activities of daily living (IADL’s). The care plan is based on an assessment of the client’s needs and is developed in conjunction with a functional assessment, with input from the client and/or the client’s family.
- Ambulation – Range of Motion – Bathing
- Personal Hygiene – Dressing – Grooming
- Feeding – Exercise – Transferring
- Positioning – Toileting and Incontinence Care – Medication Reminders
- Housekeeping – Meal Planning and Preparation – Laundry
- Transportation – Shopping and Errands – Companionship
- Vital Signs – Blood Pressure
- Ensure client’s safety and security by supervising the home environment
- Teach/Perform Meal Planning and Preparation and serving the client
- Routine Housekeeping activities such as making/changing bedding, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, and laundry (household tasks should not exceed 20% of time worked per shift unless the client has a Housekeeping Authorization)
- Provide Companionship including social interactions, conversations, emotional reassurance, and encouragement of activities that stimulate the mind
- Provide respite care for families in accordance with care plans
- Perform/assist with essential shopping/errands, handling the client’s money under the observation of the Supervisor or Administrator*
- Assist clients in following a written, special diet plan and reinforce dietary compliance as identified on the care plan
- Assist clients with standing, walking, sitting, and rolling in bed
- Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan*
- Assist clients with communication by writing or typing correspondence or researching information
- Participate on the Care Team by providing input and suggestions
- Notify agency when unable to work scheduled assignments – 24‑hour notification required
- Ensure service is delivered in accordance with all relevant policies, procedures, and practices
- Monitor supplies and resources
- Follow the written care plan and work all hours as established by Supervisor
- Carry out duties as assigned by the Supervisor
- Observe clients and their environments and immediately report unsafe conditions to Supervisor
- Observe clients and their environments and immediately report behavior, physical and/or cognitive changes and/or changes in living arrangements to Supervisor
- Complete and maintain records of daily activities, observations, and direct hours of service
- Submit written documentation to the office as required
- Attend orientation, in‑service training sessions, and staff meetings
- Develop and maintain constructive and cooperative working relationships with others
- Make decisions and solve problems
- Communicate regularly with Supervisor and co‑workers
- Observe, receive, and obtain information from relevant sources
- Report any incidents or accidents immediately to Supervisor
- Consistently follow Code of Conduct and policies as set in the Employee Handbook
- Maintain client’s protected health information confidentially and observe all client’s rights
- Report, detect, and prevent abuse and neglect
