Medical Social Worker

We are seeking a Medical Social Worker to join our home health and hospice team! You will provide emotional and administrative support to clients in either individual or group settings, generally in private homes and Assisted Living Facilities. This is a PRN position potentially growing into a part time position. Typically it is around 5-10 hours a week. Schedule is very flexible!

PRN, Weekday, Flexible Schedule, Weekend, Days or Evenings

Responsibilities:

• Meet with patients per request regarding a range of social services needs.
• Assess nature of clients' situations by interviewing and reviewing personal history
• Determine appropriate milestones to gauge client progress
• Communicate client updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs
• Maintain accurate and timely client documentation

Requirements

• Current State Social Workers License: MSW
• Previous experience in social work, counseling, or other related fields
• Compassionate and caring demeanor
• Ability to build rapport with clients
• Strong leadership qualities
• Excellent written and verbal communication skills
• BLS/CPR
