Job Overview
Provides care in a hybrid Registered Nurse (RN) role, dividing time between routine home health visits and serving as a wound care resource. This position delivers direct patient care in the home setting while also supporting specialized wound assessment, consultation, and care planning to optimize clinical outcomes.
Responsibilities• Deliver clinical nursing care during home visits, including assessment, medication administration, and documentation.
• Provide wound care assessment, consultation, and individualized care planning.
• Mentor and guide nursing staff as required.
• Maintain accurate patient records and collaborate with interdisciplinary teams.
• Support patient and family education about home health care and wound management.
- Current license to practice as a Registered Nurse (RN) in the state of practice
- Current driver’s license with insured and reliable transportation and an acceptable driving record
- Bachelor’s degree in nursing (BSN) from an accredited institution (degree will be verified)
- Registered Nurses hired or promoted into this role must obtain their Bachelor of Science in Nursing (BSN) within four years of their hire or promotion date
- Basic Life Support (BLS) Certification for Healthcare Providers
- Unit‑specific certification as required (e.g., ACLS, PALS, etc.)
- Applicants must either have required additional certifications (e.g., ACLS, PALs, NRP) upon hire or must obtain them within a specified timeframe determined by service line and department guidelines.
- One year of clinical nursing experience
- Demonstrated experience in assigned clinical area
- Demonstrated ability to mentor and guide nursing staff
- Previous home health nursing experience
- Experience in wound and ostomy care
- Wound, Ostomy, and Continence (WOCN) certification or eligibility strongly preferred
- Previous leadership experience
The role requires frequent visual and auditory engagement, manual dexterity, regular computer use, and the ability to lift, carry, and move patients and supplies, including items up to several pounds. The candidate must be able to walk, stand, bend, push, and pull equipment as needed.

Home Services – Cedar City, Utah.

Hours:
40.Hourly Rate: $40.39 – $60.96 (actual rate dependent on experience).
