About the roleMonovo is seeking an RPM Clinic Coordinator to support daily patient review, RPM data monitoring, patient communication, in-clinic onboarding, and compliance support. This role blends patient review, care coordination, communication, documentation, and hands‑on clinic support.
You will work closely with nursing teams, clinic staff, providers, and operations to make sure patient readings are reviewed consistently, trends are identified early, patients receive timely outreach when appropriate, and clinic teams have the support they need. This is a great opportunity for someone who is detail-oriented, dependable, comfortable communicating with patients, and excited to support preventive care through remote patient monitoring.
PatientReview and RPM Data Monitoring
- Review normal RPM readings across active clinics.
- Monitor patient data for changes from each patient's individual baseline.
- Identify early trends that may need patient outreach or care team awareness.
- Recognize when normal readings begin trending outside of a patient's typical range.
- Call or text patients when low-concern trends or engagement needs are identified.
- Escalate concerning patterns or abnormal trends according to internal workflows.
- Document review activity, outreach, and patient updates clearly.
- Welcome and support patients during onboarding.
- Assist patients with setup, education, and understanding of RPM tools or devices.
- Explain expectations for ongoing monitoring and communication.
- Help patients with basic device, reading, or workflow questions.
- Support clinic staff with onboarding workflows and patient readiness.
- Document onboarding completion and relevant patient notes.
- Contact patients who are missing readings or falling behind on monitoring expectations.
- Provide basic reminders related to readings, device use, and compliance.
- Identify simple barriers to compliance, such as device confusion or lack of understanding.
- Help patients stay engaged with their monitoring expectations.
- Notify the appropriate care team member when patient responses indicate a higher level of concern.
- Welcome and support patients during in-clinic onboarding.
- Assist patients with device setup, program education, application use, and monitoring expectations.
- Support clinic staff with onboarding workflows, patient readiness, and documentation needs.
- Provide basic troubleshooting support for RPM devices, readings, or workflow questions.
- Pause patient review as needed to support in-clinic patient needs, then return to review work once the patient need has been addressed.
- Maintain accurate documentation related to onboarding, patient support, and clinic coordination activities.
- Communicate patient updates, potential trends, compliance issues, and workflow concerns to nursing teams and clinic staff.
- Support continuity of care by helping organize and route information for patients who may require follow‑up.
- Follow escalation protocols consistently and promptly.
- Understand the boundaries of the role and route clinical interpretation or decision‑making to licensed clinical staff.
- Help improve review workflows, documentation consistency, and clinic support processes.
- Help track patients who are hospitalized or recently discharged.
- Follow up with patients or caregivers as appropriate.
