Travel Requirements: This role involves regular travel providing Home-Based Care in Van Buren county. (Paw Paw, Decatur, Lawrence, etc.)
What You’ll DoThe Home Care Participant Care Assistant (CNA) provides essential in-home care and assistance to participants enrolled in the PACE program. You will work closely with the Home Care Supervisor to deliver compassionate, person-centered care and support activities of daily living. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve.
