Nurse Practitioner or Physician Assistant - Physical Medicine & Rehabilitation Company Overview

Total Extended Care Services is a growing Physical Medicine and Rehabilitation (PM&R) group focused on improving patient outcomes in sub-acute and skilled nursing settings. Our team partners closely with rehabilitation professionals to help patients regain function, reduce hospital readmissions, and improve quality of life.

The Opportunity

This role offers the chance to work side-by-side with experienced PM&R physicians in a collaborative, interdisciplinary environment. It's a strong fit for a clinician who wants to focus on functional recovery, continuity of care, and measurable patient outcomes—without the burden of on-call responsibilities.

You’ll play a key role in helping patients transition successfully through rehabilitation while working with a team that values coordination, consistency, and long-term impact.

What You'll Do
  • Partner with a PM&R physician to manage patient care in skilled nursing and sub-acute rehab settings
  • Collaborate closely with therapy teams (PT/OT/ST) to align medical care with functional recovery goals
  • Coordinate with primary care providers, nursing staff, social workers, and families to ensure continuity of care
  • Provide ongoing evaluation and management with a focus on pain control, mobility improvement, and reducing hospital readmissions
  • Complete timely and accurate documentation, with flexibility to chart remotely
What Success Looks Like
  • Patients demonstrate measurable improvement in mobility, function, and overall rehabilitation outcomes
  • Reduced avoidable hospital readmissions through proactive care and coordination

Strong alignment between medical treatment plans and therapy goals
  • Consistent, timely documentation that supports high-quality, compliant care
  • Positive collaboration with interdisciplinary teams across multiple facilities
What We're Looking For
  • Licensed Nurse Practitioner or Physician Assistant (required)
  • Active certification as an NP or PA (required)
  • Current Michigan license (required)
  • Ability to work independently while collaborating across disciplines
  • Strong communication skills and a patient-centered approach to care
  • Reliable transportation to travel between facilities
Compensation and Benefits
  • Flexible schedule
  • No on-call responsibilities (evenings and weekends free)
Work Location / Schedule
  • In-person at skilled nursing facilities
  • Flexible scheduling options available
  • Part-time opportunities available
