Compensation: $19.38 - $21.39 / hour

Schedule: Full-time 40 hours per week | 8am - 4:30pm | Monday - Friday

Travel Requirements: This role involves regular travel providing Home-Based Care.
You must live in Van Buren County for this position to be a feasible commute.

What You’ll Do: The Home Care Participant Care Assistant (PCA) provides essential in‑home care and assistance to participants enrolled in the PACE program. You will work closely with the Home Care Supervisor to deliver compassionate, person‑centered care and support activities of daily living. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve.

• Provide Hands‑On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping.
• Provide Responsive Support – Answer call lights quickly and respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top‑

• One (1) year of prior experience with frail or elderly individuals, aiding with daily living activities.
• Valid Michigan Driver’s License with a clean driving record and reliable transportation.
• CNA certification, desired.
• Completion of the Participant Care Assistant (PCA) program at PACE of Southwest Michigan or an equivalent training program if not a Certified Nurse Assistant.
• Current CPR or BLS certification or willing to obtain.
• Ability to work independently, multitask, and operate as part of an interdisciplinary team.
