PACE of Southwest Michigan is seeking an LPN Home Care Coordinator in Saint Joseph, MI to coordinate nursing services for participants in homes and community settings.
You will complete participant visits, provide nursing support within the LPN scope, communicate changes in participant condition, and help ensure care plans and services are carried out effectively for PACE participants. The role is full-time with 40 hours weekly and requires home visits within the service area.
