• The Mental Health Therapist works under the supervision of the Practice Manager to provide clinical assessment, referral and/or counseling for mental health problems. This position is employed by Mosaic Life Care.

• Provide assessment, diagnosis, counseling, and follow-up to clients. Maintains and interprets documentation of clinical work and other statistically relevant data; meeting quality assurance standards.
• Provides assessment, counseling and/or referral services to patients at Mosaic Outpatient Behavioral Health or Mosaic Counseling Care. Provides assessment, counseling and/or referral services to patients at Mosaic Outpatient Behavioral Health or Mosaic Counseling Care.
• Provide computerized/written reports and/or clinical documentation for legal purposes and administrative accountability.
• Attend relevant and required meetings to provide consultation and gain needed information relevant to work.
• Other duties as assigned

• Master's Degree in Counseling or Master's Degree in Social Work is required.
• Case Mgmt/Social Services CSW
  - Licensed Clinical Social Worker - State Licensure Licensed Clinical Social Worker is required.
  
  Clinical PC - Licensed Prof Counselor is required.
  
  C.E.A.P./A.A.M.F.T. certified is preferred.
• 3 Years of Clinical experience is preferred.
