Job SummaryThe Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient's hospital stay and coordinates planning with other disciplines utilizing a patient/customer driven approach.
Core Responsibilities- Performs comprehensive nursing assessment and reassessment.
- Performs age-appropriate admission or transfer assessment and obtains input from family or guardian when appropriate.
- Accurately documents findings in the medical record.
- Performs assessment of post-op or post-invasive procedure patients.
- Assesses and documents education and discharge needs of patient and family on admission and throughout hospitalization.
- Provides patient reassessment and documents pertinent observations according to the patient plan of care, changes in condition, status or diagnosis, response to care, procedures, etc., and standards of care.
- Establishes, coordinates and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols, and standards of care.
- Identifies short- and long-term goals based on patient care needs.
- Formulates nursing interventions to achieve desired patient outcomes.
- Incorporates disease-specific evidence-based practice into nursing care and documentation.
- Provides and documents nursing interventions based on assessed patient needs, plan of care, and changes in patient status.
- Collaborates with health team members to coordinate daily plan of care for assigned patients.
- Provides, coordinates and communicates patient care, including accurate handoff communication reports.
- Administers medications accurately and documents them according to policies.
- Monitors and documents accurate IV fluids and parenteral nutrition.
- Completes referrals as indicated by assessment data.
- Requests consultation for special needs, equipment or information for patient or family.
- Provides patient and family education and discharge planning per documentation guidelines and protocols.
- Clarifies physician orders as warranted and acknowledges and implements them accurately and timely.
- Assists physician with procedures or delegates to care partner as appropriate.
- Documents “Readback” for all telephone or verbal orders and takes such orders only in emergency situations.
- Recognizes changes in patient condition and takes appropriate nursing actions.
- Involves family or guardian when providing care or decision-making is appropriate.
- Recognizes risks for the patient and takes appropriate action.
- Incorporates infection control practices daily.
- Documents and/or communicates nursing care and changes in patient condition.
- Evaluates effectiveness of care and nursing interventions, education, and updates the plan of care as needed.
- Recognizes significant changes in patient clinical parameters and reports immediately to physician or others as indicated.
- Identifies problems, gathers data, suggests solutions, and works toward problem resolution.
- Reports variations from care or treatment per policy and procedure.
- Provides specialized care to high-risk patients.
- Restraint care: initiates, evaluates, and applies restraints as approved; monitors patient response; provides specified care; consults peers; documents use thoroughly.
- Pain management: assesses presence of pain, incorporates beliefs, implements prevention-focused techniques, and includes family in plan.
- Abuse assessment: recognizes signs, reports to appropriate parties, supports patient safety.
- Adheres to quality, performance, policies, procedures, and protocols in all practice.
- Participates in learning experiences, demonstrates technical and cognitive skills, stays current with hospital information and policies, attends staff meetings, and engages in committees.
- Provides supervision, teaching, and orientation of new staff or students.
- Maintains safety and infection control guidelines, including standard precautions, hand hygiene, safe patient handling.
- Maintains a clutter-free, clean, therapeutic environment.
- Respects dress code, reports risk concerns.
- Completes mandatory annual requirements, including licenses and certifications.
- Delivers service excellence: prompt, respectful, courteous, meets patient and family needs, collaborates, communicates, offers constructive feedback, manages resources effectively, upholds hospital principles.
- Participates in performance improvement activities, develops unit-based improvement plans, leads teams when requested, and contributes to nursing research.
- Provides age-appropriate care tailored to adult or geriatric patients.
- Applies cost-effective practices, documents supplies, and manages resources to reduce waste.
Minimum Education: Licensed to work as an RN in the state of Missouri.
