In-Home Nurse Practitioner Part Time - West Plains

Job Summary

Signify Health, part of CVS Health, seeks a Part‑time Nurse Practitioner to deliver in‑home health evaluations in members’ homes. The role focuses on patient‑centered care, collaborative relationships, and bridging gaps in health delivery across a national network.

• Conduct in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical examination, and diagnostic procedures as ordered by the member’s health plan.
• Use an iPad and an intuitive clinical workflow to deliver each evaluation; not responsible for prescribing medication, ordering lab tests, or altering current treatment regimens.
• Build trusted relationships with patients, deliver personalized care plans, and connect members to appropriate providers or services.
• Work collaboratively with Signify Health clinician colleagues and fulfill the requirements of the specified role.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams, DEE).

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi‑state licenses preferred.

Anticipated weekly hours: 29.5. Part‑time. Pay range: $37.66–$81.11 per hour (base rate); actual base salary will depend on experience, education, geography and other factors.
