Visiting Nurse Association of Southeast Missouri is seeking an LPN to provide skilled hospice care in patients’ homes, nursing homes, or private facilities. You will perform nursing observations, documentation, and follow CMS regulations under the supervision of R.N.
Staff, with participation in IDG meetings and timely updates to the care plan. The role includes on-call rotations, EMR documentation, medication reconciliation, and collaboration with physicians to adjust care as conditions change.
