Position: Employee Health Services RN Case Manager

Overview

Health Partners is hiring an Employee Health Services Case Manager. This position is responsible for assessing needs, coordinating services, providing support and guidance to Health Partners colleagues across the Organization in addressing colleague health and safety needs. This includes proactive Employee Health Services programs support and incident intake and management to resolution via phone, voicemail, email, online chat, and other routes.

The role balances stakeholder needs (e.g., colleagues, leaders, organization, OSHA, MDH, Joint Commission) and takes ownership to ensure colleagues receive resolution and access to appropriate resources.

Accountabilities
  • Responsible for conducting colleague exposure investigations
  • Evaluates colleagues’ risk of exposure
  • Provides nursing expertise for the worker’s compensation program
  • Assesses colleague’s return to work status
  • Coordinates with applicable stakeholders
  • Provides nursing expertise for the ergonomic program
  • Assesses medical questionnaires submitted as part of the request for an ergonomic evaluation
  • Provides nursing expertise for the respiratory protection program
  • Assesses medical questionnaires submitted by colleagues for fit testing
  • Provides nursing expertise for medical surveillance programs
  • Assesses medical questionnaires provided by colleagues enrolled in the program
  • Provides nursing expertise for the colleague vaccine program
  • Educates colleagues regarding vaccine safety
  • Provides nursing expertise for colleague leaves of absence
  • Evaluates ADAAA medical questionnaires and reasonable accommodation requests
  • Evaluates FMLA medical certifications
  • Provides nursing expertise in new hire screenings
  • Evaluates colleagues’ lab results

  • Is a confident guide to support colleagues in complex and stressful situations
  • Identifies interventions and resources to assist colleagues in reaching health-related goals
  • Identifies patterns, episodes, and gaps predictive of future needs and services
  • Protects the confidentiality of colleague health records and balances regulatory/organizational requirements
  • Integration of clinical and psychosocial information for action-oriented planning
  • Facilitates integration of care through effective communication among colleagues, leaders, providers, and other parties
  • Ensures compliance with organizational and outside agency requirements and regulations
  • Maintains current and accurate documentation and confidentiality
  • Works collaboratively to ensure timely, compassionate, effective service
  • Maintains knowledge of IT applications and systems; seeks training as needed
  • Identifies deficits in technological literacy and pursues appropriate training
  • Other duties as assigned; may require evening, weekend, holiday, and on-call coverage to meet needs
Required Qualifications
  • Current Registered Nurse in the State of Minnesota
  • Current Registered Nurse in Wisconsin within 2 months of hire
  • BSN or three years RN experience
  • Demonstrated independent nursing judgment and skills
  • Demonstrated critical thinking and problem-solving skills
  • Demonstrated ability to collaborate with care team members using strong written, oral, and interpersonal communication
  • Working knowledge of quality improvement
  • Ability to adapt to challenging situations with flexibility and organization
  • Patient-centered approach
  • Robust computer skills, including Microsoft Office; eagerness to learn new systems
Preferred Qualifications
  • Previous experience as a case manager or in Employee Health, Occupational Medicine, or related area
  • Master’s degree in related field
  • Certified Occupational Health Nurse (COHN)
Decision-making
  • Makes independent decisions within scope and determines when to consult leadership or Medical Director
  • Uses clinical judgment, organizational knowledge, and critical thinking to determine plans for colleagues; consults leadership/Medical Director when indicated
  • Makes recommendations to leadership regarding policy development or changes
  • Clinical supervision is provided by the Employee Health Education and Training Specialist
