The Assistant Director of Health Services Nurse is responsible for ensuring that the safety and health needs of the senior clients are met while providing staff leadership duties.
Schedule- 80 hours every two weeks (Full Time)
- 8:00 a.m. – 4:30 p.m., Days

- Maintain current assessment and documentation of client’s health status.
- Perform initial and ongoing assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensure RN assessment is completed upon admission, within 14 days of admission, and at least every 90 days, and when/if there is a change in health condition.
- Work with client and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement.
- Assure that clients have up‑to‑date service agreements. Implement service agreements with new tenant/residents. Review and update service agreements at least once per year and more frequently when there is a change in service needs.
- Develop, maintain, and implement individual emergency plans based on health status and Advance Care Planning documents, including POLST.
- Maintain clinical charts and progress notes using computerized documentation system.
- Communicate changes in client’s conditions to medical and other relevant individuals.
- Coordinate client health services.
- Facilitate coordination of services with internal and external providers.
- Ensure that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.
- Work with client and/or representative(s) to assure delivery of appropriate services and funding.
- Participate in care conferences as needed or requested by the DHS or Executive Director.
- Vocational School / Technical Training or Associate’s Degree in Nursing
- One to Three Years of Experience
- MN Registered Nurse (RN) License
- Basic Life Support (American Heart Association or Red Cross)
- Bachelor of Science in Nursing
- Three to Five Years of Experience
- Prior supervisory experience
- Prior experience in home care or assisted living environment
