Bluestone Physician Services is seeking a Care Manager to support complex, chronic patients in senior living communities across Minnesota. This field-based role links patients, families and the primary care provider to execute unified care plans and reduce hospitalizations.
Work schedule is 8am–5pm Monday–Thursday and 8am–3pm Fridays, with 90% field-based patient care across Minnetonka, Golden Valley, Edina, Plymouth, Orono, and St Louis Park.
