Park Nicollet Hospice is hiring a Registered Nurse to join the outpatient hospice team as a Case Manager. You will assess patient needs, provide skilled nursing care and teaching to patients and families in their home or community, and coordinate resources to develop a comprehensive plan of care.
The role supports the West Team (e.g., Lake Minnetonka area) with a schedule of 4 days a week, 8:00 am to 4:30 pm, and every 4th weekend.
