Ever True PACE is seeking a compassionate Primary Care Physician to join our interdisciplinary care team in St. Louis.
This role offers the opportunity to deliver relationship-based, value-driven care to older adults—without the volume pressures of traditional primary care.

- Value-based, outcomes-focused care
- True interdisciplinary team model
- Continuity of care across settings
- Provide primary and continuous medical care to PACE participants
- Collaborate closely with Nurse Practitioners and the interdisciplinary team
- Coordinate care with specialists, hospitals, and skilled nursing facilities
- MD or DO;
Board Certified in Internal Medicine or Family Medicine - Active Missouri Physician License, DEA, and CDS
- Experience caring for an elderly population
- Passion for team-based, value-driven care
