Job Summary: The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient's hospital stay and coordinates planning with other disciplines utilizing a patient/customer driven approach.

Responsibilities
  • Performs comprehensive nursing assessment and reassessment.
  • Performs age-appropriate admission or transfer assessment, obtaining input from family/guardian when appropriate.
  • Accurately and completely documents findings.
  • Assesses and documents education and discharge needs of patient and family on admission and throughout hospitalization.
  • Provides patient reassessment, documenting pertinent observations according to the patient plan of care, changes in condition or diagnosis, response to care, procedures, etc.
  • Establishes, coordinates, and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols and standards of care.
  • Identifies short and long term goals based on patient care needs.
  • Formulates nursing interventions to achieve desired patient outcome and incorporates disease-specific evidence-based practice into nursing care plan and other documentation.
  • Collaborates with appropriate health team members for coordination of daily plan of care for assigned patients and provides, coordinates, and communicates patient care, including accurate handoff communication reports.
  • Administers and documents medications accurately according to policies and procedures; monitors, maintains and documents accurate IV fluids and parenteral nutrition accordingly.
  • Completes referrals as indicated, requests consultation for special needs, equipment, or information for patient and/or family.
  • Provides patient and family education and discharge planning per documentation guidelines and protocol.
  • Clarifies all physician orders as warranted; acknowledges and implements orders in an accurate and timely manner, and documents "Readback" for all telephone or verbal orders.
  • Recognizes changes in patient condition and takes appropriate nursing actions; involves family/guardian in care and decision-making when appropriate.
  • Incorporates infection control practices in daily care; documents and or communicates nursing care and changes in patient condition.
  • Evaluates effectiveness of care, patient/family education, and modifies plan of care as indicated.
  • Recognizes significant changes in patient's clinical parameters and reports immediately to physician and others as indicated; works toward problem resolution and reports variation from care or treatment following reporting policy.
  • Provides specialized care to patients at high risk for injury, including restraint care, pain management, and abuse assessment, and follows appropriate protocols.
  • Adheres to quality and performance standards, policies, procedures, and protocols, and participates in learning experiences to increase professional competence.
  • Maintains currency in all hospital/unit information, communication, policies and procedures; attends staff meetings and participates in committees, governance, and shared‑governance activities.
  • Provides assistance and support to peers and co‑workers, adapts to changing work demands, and maintains professional relationships.
  • Orients, precepts, and participates in collaborative teamwork, functioning as a resource person to co‑workers, patients/families, medical staff and other team members; delegates care and activities to appropriate team members.
  • Performs safety and infection control practices, including standard precautions, disposal of hazardous wastes, hand hygiene, safe patient handling, and maintains a clean, safe environment.
  • Assumes responsibility for completing annual mandatory requirements such as licensure, certifications, BLS/HT, TB screening, influenza vaccination, ethics training, safety training, information security, abuse & neglect, blood‑borne pathogens, and competency assessments.
