Nurse Navigator - CV Surgery - Mercy South

Youll serve as a trusted guide for patients and familiescoordinating appointments, providing pre and postop education, triaging calls, and ensuring every patient feels supported and informed. Youll also collaborate with surgeons, APPs, and inpatient teams to keep care plans moving smoothly.

Cardiothoracic Surgery

Location:Mercy South
Schedule:Full-Time | 40 hours/week | Monday through Friday

Key Responsibilities:

• Provides comprehensive pre and postoperative patient education, drives quality efforts to reduce readmissions, and manages procedure scheduling.

• Implement structured postprocedure tracking to reduce readmissions.

• Support daily CV clinic operations within budget and staffing guidelines.

• Prepare the department for increased patient volume, including the arrival of a new CV surgeon.

• Strengthen quality outcomes, patient access, and provider support.

• Provide CV casescheduling support as procedural volume grows.

• Offer cross coverage within the CV clinic to maintain patient flow and operational efficiency.

• Offload nonprocedural and care coordination tasks from APPs to enhance access and allow topofscope practice.

Qualifications

• Licensure: Current RN (Registered Nurse) license in state of Missouri.
