Position summary:
perform the duties required to coordinate and monitor nursing services for the chronic disease department. Provide direct care with patients, work with health coaches and bcccp follow-up assistant.
Position qualification requirements
Education:
bachelor of science degree in nursing.
Experience:
* five (5) years' experience in ambulatory care, hospital and /or community health nursing.
* experience in ambulatory care, hospital or community health nursing.

* good organizational, analytical, and problem-solving skills.
* excellent verbal and written communication skills.
* able to work with people with diverse ethnic backgrounds.
* strong computer skills including the use of word process, spreadsheet and e-mail software.
* able to work on multiple tasks simultaneously.
Residency and transportation requirement: must possess a valid driver's license and automobile with insurance coverage as required by the state of missouri.
License(s)/certification(s) required: licensed rn in missouri and cpr certified.
Functions of the job
Essential functions
* work with health coaches' compliance to established policy and procedures with daily work schedules.
* recruit, patients with chronic disease conditions and work with qualified health coaches and bcccp follow-up assistant.
* coordinate and implement training, educational classes and in-services for staff in chronic disease management, patients and jacho standards.
* assist in the development and implementation of policies and procedures for chronic disease department and other areas as required.
* coordinate and oversee special programs as assigned.
* prepare and maintain needed records and reports.
* prepare and revise educational materials for clients with chronic diseases.
* complete outreach/in reach and follow up activities provided through bcccp.
* participate as a member of the health disparities collaborative teams dealing with chronic diseases.
* work in coordination with health center nurse coordinators and supervisors to monitor and maintain adherence to jcaho and other accrediting body standards.
* represent affinia healthcare in the community and make presentations as requested.
* monitor the goal achievement of healthy heart program and other grants and contracts involving coaching of clients with chronic diseases.
* participate in the planning and evaluation of new and existing programs.
* perform other duties as assigned.
Marginal functions: orientates volunteers, physicians, nurse instructors and students.
