MDS Coordinator

Join us as an LPN / RN MDS Coordinator, playing a pivotal role in maintaining our exceptional standards. We are seeking an experienced MDS coordinator to join our Management team as well as learn and grow with our high-level upper management support.

• You Will Play a Critical Role:
  As an LPN /RN MDS Coordinator, you'll conduct and coordinate resident assessments, ensuring compliance with federal, state, and local standards.

  Collaboration:
  
  Work closely with the Interdisciplinary Care Plan Team to develop comprehensive care plans for each resident.

• Resident Assessment Development:
  Conduct and coordinate Resident Assessment Instruments (RAIs) in line with regulations and guidelines.
• Supportive
  
  Collaboration:
  
  Collaborate with the Care Plan Team to craft personalized resident care plans.
• Evaluation and Reporting:
  Assess resident conditions, ensuring special assessments are conducted when necessary, and provide necessary data for the Facility's Quality Assurance Program.

• Active LPN/RN MO License:
  Current and in good standing.
• MDS Coordinator
  
  Experience:
  
  Prior experience in a similar role.
• Regulatory Knowledge:
  Familiarity with Medicare/Medicaid regulations and benefit guidelines.
