Position: Nurse Care Manager- Full Time

Job Description

Nurses: Are you looking to focus on care and quality of life, not paperwork? Martha's Hands is expanding its team of Nurse Care Managers for Private Pay Home Care clients.

Responsibilities
  • Utilize your nursing skills as part of a 12‑person Care Supervisory Team.
  • Guide a client and their family through their care needs.
  • Support and train caregivers in providing direct care to clients in their homes.
  • Partner with home health, hospice and other care professionals to manage care.
  • Conduct assessments with clients and families to develop a customized service plan.
  • Coordinate care with other healthcare professionals including home health, hospice, primary care, etc.
  • Orient and train caregivers to provide excellent care.
  • Provide clinical oversight and support to caregivers.
  • Provide direct care to clients – perform skilled nursing tasks and ADL assistance as needed, including medication management, wound care, diabetic care, tube feedings.
  • Assist families with more detailed care management services as needed.
  • Balance an office and mobile work environment, utilizing technology to enhance care and efficiency.
  • Supervise the care team to ensure quality care expectations are met while inspiring and encouraging caregivers.
Qualifications
  • 3+ years nursing experience; preferred experience in private duty, home health, hospice, geriatric nursing or medical‑surgical nursing.
  • Team‑oriented approach, strong professional and interpersonal communication skills.
  • Strong clinical assessment skills to develop customized service plans directing client care.
  • Educational and training abilities to enhance caregiver success in caring for clients.
