Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) | Full-Time

• Monitor health conditions and ensure quality nursing care is delivered.
• Administer medications and maintain accurate medical documentation.
• Oversee and support staff responsible for medication administration.
• Provide treatments and personal care according to physician orders.
• Conduct health assessments and implement care within each Individual Service Plan (ISP).
• Recognize and respond to changes in health conditions.
• Train staff on infection control, universal precautions, and medication administration.
• Coordinate care with physicians, healthcare providers, and external agencies.
• Educate individuals served on health topics such as self‑medication and prescribed treatments.

• Associate's Degree in Nursing or related field, or state certificate.
• Active LPN/LVN license in the state of Missouri.
• At least one year of experience working with the population served in a nursing capacity.
• Valid driver's license, car registration, and auto insurance.
• Other certifications as required by state licensing regulations such as CPR/FA, HIV/AIDS certification, BBP, and ACT training.
• Ability to effectively communicate with staff, individuals served, and families.
