Licensed Practical Nurse; LPN

The LPN is a medical professional cross‑trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of an LPN vary from setting to setting, depending on the size, location and type. In addition to administrative duties, LPNs perform clinical duties under direct physician supervision in accordance with state medical practice acts.

• Measure and record patient vital signs; record patient interview and medical history.
• Catalog and communicate patient needs to the PCP, Clinician, Specialist and/or other clinical personnel.
• Prepare patients for examinations and perform routine screening tests; assist the physician with exams; explain treatment procedures and physicians' instructions to patients.
• Observe patients and report any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel.
• Perform non‑critical medical and therapeutic procedures based on medical instructions.
• Collect phlebotomy and other lab specimens; perform basic waived lab tests.
• Prepare and administer medications; change dressings, apply bandages, remove sutures and other first‑aid procedures; use CPR skills when necessary, all under physician supervision.
• Maintain supplies, equipment, stocks and sterilize instruments; practice OSHA safety standards; perform accurate, legal and ethical documentation at all times.
• Process patient phone messages, returning calls and routing them to other team members as appropriate; call patients to obtain and relay pertinent information for the physician.
• Upon physician approval and authorization, call in prescriptions to the pharmacy. Use communication skills with appropriate medical terminology; and follow appropriate legal and ethical professional conduct; authorized individuals will use the Chen Med Rx system to support physician medication dispensing.
• Provide health coaching to a defined group of patients to support healthy lifestyle choices; follow up with coached patients via weekly calls.
• Perform other duties as assigned and modified at manager’s discretion.

• Proficient skills in Microsoft Office Suite products including Word, PowerPoint, Outlook and Excel plus a variety of other word‑processing, spreadsheet, database, e‑mail and presentation software.
• Spoken and written fluency in English.
• Ability and willingness to travel locally and/or regionally up to 10% of the time to assist in covering other centers, as needed.

Pay Range: $20.20 – $28.83 Hourly
