Job Summary
Registered Nurse (RN) – PRN – responsible for assessing, managing, directing, and providing patient‑focused nursing care during hospital stay. The RN coordinates care with interdisciplinary teams and utilizes a patient/customer driven approach.
Essential Responsibilities- Reports risk management concerns using eSRM.
- Completes Annual Blood Borne Pathogens training (if required).
- Performs comprehensive nursing assessment and reassessment.
- Conducts age‑appropriate admission or transfer assessments and obtains input from family/guardian when appropriate.
- Accurately and completely documents findings including post‑op and post‑invasive procedure patients.
- Assesses and documents education and discharge needs of patient and family on admission and throughout hospitalization.
- Provides patient reassessment documenting pertinent observations according to the patient plan of care, changes in condition, status, diagnosis, response to care, procedures, and standards of care.
- Establishes, coordinates and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols, and standards of care.
- Identifies short‑ and long‑term care goals based on patient needs.
- Formulates and documents nursing interventions to achieve desired outcomes and incorporates evidence‑based practice into care plans.
- Collaborates with health team members to coordinate daily plan of care for assigned patients.
- Communicates patient care, including accurate handoff reports, and documents medication administration and IV fluids per policies and procedures.
- Monitors, maintains, and documents IV fluids and parenteral nutrition accurately.
- Completes referrals, requests consultations, and provides patient/family education and discharge planning per guidelines and protocols.
- Clarifies physician orders when necessary and implements orders accurately and timely.
- Assists physician with procedures or delegates to care partner as appropriate.
- Records “readback” for telephone or verbal orders and takes such orders only in emergency situations.
- Recognizes changes in patient condition and takes appropriate nursing actions, involving family/guardian in decision‑making as needed.
- Recognizes patient risks and applies infection control practices daily.
- Documents and communicates nursing care and changes in patient condition.
- Evaluates effectiveness of care based on assessment data, interventions, medication response, and procedures.
- Evaluates patient/family education effectiveness and modifies plan of care as needed.
- Reports variations from care/treatment per reporting policy and develops problem‑solving communication strategies.
- Provides specialized care to high‑risk patients, including restraint care, pain management, and abuse assessment.
- Maintains quality and performance standards, policies, and procedures during clinical care.
- Participates in continuous learning to increase professional competence and stays updated on hospital/unit information.
- Adapts to changing work demands, supports peers, and demonstrates professional courtesy and problem‑resolution engagement.
- Orients and precepts new employees when required and participates in collaborative team approaches.
- Acts as a resource person for coworkers, patients/families, and medical staff, including supervising students or new staff nurses.
- Practices safety and infection control, disposes of hazardous waste, maintains a clean environment, uses safe patient handling techniques, and adheres to dress code.
- Completes mandatory training: current license and certifications, BLS and HI, annual TB screening, influenza vaccination, ethics training, safety training, information security, abuse & neglect course, blood‑borne pathogens training, and competency activities.
- Delivers customer‑centric service with respect, empathy, and effective communication, maintaining confidentiality and assisting with patient requests promptly.
- Supports unit performance improvement, participates in data collection, and leads improvement projects as needed.
