Find your calling at Mercy! The Nurse Navigator is a leader providing support, mentorship, and guidance to those who provide direct patient care. The Nurse Navigator coordinates the delivery of patient care by assessing care requirements of the individual patient and makes recommendations based on severity of illness, intensity of service and skill level. The Nurse Navigator ensures patients receive timely, safe, effective and high quality care.
They coordinate the patient’s plan of care with the bedside nurse and set expectations regarding professional conduct. The Nurse Navigator holds direct and indirect care providers accountable for their professional responsibilities and adherence to regulatory guidelines. The role supports an effective new care model, promoting integration of a seamless care model, coordinating patient discharge planning for post‑acute services, length of stay (LOS) management and utilization management, and collaborating to optimize performance and adoption of best practice.
- Pre‑op education
- Daily rounding while the patient is in the hospital
- Patient discharge disposition
- Follow‑up care
- Coordinate patient care plans with bedside nurses and other providers
- Collaborate with interdisciplinary teams to optimize care transitions
- Graduate from an approved school of nursing
- Current RN license in state of practice
- 2‑3 years of experience in a clinical nursing role
- BLS (Basic Life Support) certification required
- Bachelor of Science in Nursing (preferred)
- ACLS (Advanced Cardiac Life Support) certification preferred
- Pushing, pulling, or lifting up to 50 pounds
- Prolonged standing and walking during each shift
- Ability to grip, reach, bend, kneel, twist, and squat to perform duties
