Transitional Care Unit Registered Nurse

Job Summary

Registered Nurse (RN) – Transitional Care Unit (TCU) – Ongoing staff position at BJC Health Care, Saint Louis, Missouri.

• 3x12 hour shifts
• Every 3rd weekend
• Night shifts
• Sign-on Bonus up to $15k eligible for external applicants

• Provide direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation in accordance with nursing standards.
• Promote patient and family centered care in a healing environment.
• Educate patients and families on illness management, post-treatment home care needs, and medication administration.
• Participate in patient safety, quality, and regulatory compliance activities.
• Engage in professional development activities.
• Develop, implement, and document individualized plans of care with defined goals in collaboration with the interprofessional team, patients, families or caregivers.
• Proactively plan and ensure communication of the plan of care across the continuum of care.
• Use critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to the standards of care.
• Assess patient preferences and barriers to care, including values, emotional, spiritual, cultural, and population-specific needs.
• Maintain constant state of alertness and work safely as a safety-sensitive position.

Minimum Requirements

• Nursing Diploma or Associate’s Degree in Nursing
• Current RN license
• No prior clinical or supervisory experience required

Preferred Requirements

• Bachelor’s Degree in Nursing
• ED and/or inpatient experience
• 2 years of experience or less
