Job Summary:

Join our compassionate and collaborative team as a Nurse Care Manager, where you will play a critical role in supporting individuals with complex medical and behavioral health needs. This position offers the opportunity to make a meaningful impact by partnering with internal and external treatment teams to provide education, coordination, and ongoing support for clients at risk for or managing chronic health conditions.

If you are a clinically skilled nurse with a passion for integrated care, this role allows you to improve health outcomes and support long-term  this role, you will coordinate care, develop treatment plans, provide education, and collaborate across disciplines to ensure comprehensive, patient-centered support.

Key Responsibilities:

Collaborate with internal and external treatment team members to coordinate client care Participate in development and management of individualized healthcare treatment plans

Provide education to clients regarding chronic health conditions, treatments, and medications

Facilitate health education groups using advanced clinical knowledge

Support hospital admissions and discharges, ensuring continuity of care Monitor client health outcomes and track performance measures using clinical systems

Develop wellness and prevention initiatives to improve overall client health

Consult with team members regarding client health conditions and care strategies

Implement clinical technology platforms and reporting systems as required

Respond appropriately to medical or behavioral health crises

Participate in required meetings, quality assurance activities, and program evaluations

Maintain accurate and timely documentation in accordance with policies and standards

Perform other duties as assigned

Education, Experience, and/or Credential

Qualifications:

Active Registered Nurse (RN) license required

Experience working with individuals with intellectual and developmental disabilities (IDD) preferred

Additional Qualifications:

Strong knowledge of chronic disease processes across the lifespan

Excellent communication and interpersonal skills

Strong critical thinking and decision‑making abilities

Ability to work collaboratively with multidisciplinary teams

Ability to implement and adapt to new systems and processes

Strong organizational and time‑management skills

Ability to maintain confidentiality and professionalism

Physical Requirements Ability to exert up to 10 pounds of force occasionally

Ability to perform repetitive hand, finger, and arm movements for typing and documentation

Ability to sit for extended periods with occasional walking or standing

Ability to meet the physical demands of a sedentary work environment
