This position is assigned to the Department of Health
- Communicable Disease section under the general supervision of the Health Services Manager I and the Public Health Nurse Supervisor. The Public Health Nurse II will support the Tuberculosis program and be responsible for clinical duties and case management of patients with tuberculosis, primarily those with Latent Tuberculosis Infection, in addition to clinical duties. As needed, this role may require a flexible schedule, with some work hours during evenings and weekends.
- Ensure patients receive their medication and refills.
- Check in with patients routinely for medication adherence and tolerance.
- Coordinate follow-up with physicians.
- Document patient interactions in the electronic health record.
- Perform tuberculosis case investigations by collecting data through medical record reviews, interviews and site visits, as needed.
- Coordinate disease prevention activities to reduce disease transmission.
- Provide medical information, education, and consultation on communicable disease identification, treatment, and prevention to healthcare providers, educators, organizations, and the public through outreach and engagement efforts.
- Counsel infected persons on treatment, methods for prevention and importance of testing to reduce the spread of disease.
- Assist in the development and delivery of health care plans for patients.
- Provide direct client care services including vital signs, administering medications, assisting with procedures, drawing blood, patient education, and monitoring patient conditions.
- Coordinate patient referrals to other health care and social service agencies.
- Maintain complete records, forms and case reports.
- Complete weekly, monthly, and annual reports.
- Assist with outbreak investigations and reports, as needed.
- Participate and attend meetings related to communicable diseases.
- Participate in the incident command system as required for responding to communicable disease emergencies or public health emergencies.

- Data Utilization:
Requires the ability to perform mid-level data analysis including the ability to audit, deduce, assess, conclude and appraise. Requires working knowledge of MS Office computer applications (Word, Excel, PPT) and Google computer applications (Drive, Sheets, Docs). Requires working knowledge of Electronic Health Records. - Human Interaction:
Requires the ability to understand and explain the application of policies, procedures, and standards to specific situations. Requires the ability to provide first-level professional medical administration and registered nursing care. Requires the ability to use language tools to communicate effectively with non-English speaking patients. - Equipment, Machinery, Tools and Materials Use:
Requires the ability to operate, maneuver and/or provide simple but continuous adjustment on equipment, machinery, and tools such as motor vehicle, computer terminal, photocopier, fax machine, calculator and/or materials used in performing essential functions. - Verbal Aptitude:
Requires the ability to utilize a variety of reference, descriptive, and advisory data and information such as public health statistical reports, lab reports, patient records, time sheets, personnel policies, attendance records, work schedules, activity reports, public health manuals, Physician’s Desk Reference, nursing standards/manuals, disease control manuals, ordinances, statutes, regulations, procedures, guidelines, and non-routine correspondence. - Mathematical Aptitude:
Requires the ability to perform addition, subtraction, multiplication, and division; calculate fractions, percentage, and decimals; interpret and develop descriptive statistical reports. - Functional Reasoning:
Requires the ability to exercise independent judgment to apply facts and principles for developing approaches and techniques to problem resolution. - Situational Reasoning:
Requires the ability to exercise the judgment, decisiveness, and creativity required in situations involving the evaluation of information against sensory, judgmental, and/or subjective criteria, as opposed to criteria that are clearly measurable or verifiable. - Environmental Factors:
Tasks may risk exposure to adverse environmental conditions, such as toxic/poisonous agents, potential violence, disease, or pathogenic substances. - Physical Requirements:
Requires the ability to lift objects such as files and medical supplies weighing up to twenty (20) pounds. Requires the ability to stoop, bend, and twist to perform phlebotomy and TB skin tests and stand to examine patients. - Sensory Requirements:
