Nurse Navigator - Heart Failure Clinic, PRN

Find your calling at Mercy! The Nurse Navigator provides coordination and support for patients with heart failure across the continuum of care. This role focuses on assessing patient needs, delivering education, and coordinating care plans with providers and interdisciplinary teams to ensure timely, high-quality outcomes.

Nurse Navigator – Heart Failure Clinic (PRN) at Mercy Hospital South. Supports patients with heart failure and their families throughout evaluation, treatment, and ongoing management, ensuring a seamless outpatient experience.

• Assess patient needs and develop individualized, evidence‑based care plans.
• Deliver education to patients and families and facilitate follow‑up.
• Coordinate care plans with providers and interdisciplinary teams.
• Monitor patient progress, facilitate transitions of care, and ensure adherence to treatment plans.
• Track outcomes, quality metrics, and participate in process and quality improvement initiatives.
• Maintain accurate and timely documentation in EPIC and other systems.
• Enhance patient access, education, and engagement; support program growth and community awareness.

• Graduate from an approved school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification required; ACLS certification preferred.
• Bachelor of Science in Nursing (BSN) preferred.

• Ability to push, pull, and/or lift up to 50 pounds.
• Prolonged standing and walking throughout the shift.
• Ability to bend, reach, kneel, twist, and squat as needed.
