ScheduleMonday-Friday 8-16:30, weekends based on department needs.
Role PurposeActs as a patient advocate by providing education, support and interventions throughout the continuum of patient care including pre and post hospitalization. Performs duties by working with physicians to identify and carry out treatment plans.
Responsibilities- Works with the Medical Director and other physicians to identify and carry out the treatment plan.
- Acts as a Patient Advocate helping to navigate patients through the diagnosis, treatment and rehabilitation process.
- Serves as a resource to assist patients and families, other professionals and the community.
- Provides education for the community and referring physicians.
- Facilitates clinical protocols established by Medical Directors.
- Maintains patient records, monitors lab values, test results and progress, and coordinates patient discharge needs to ensure a smooth transition from hospital to outside agencies.
- Provides patient and family education.
- Conducts new patient interviews and assesses patient function, health, and educational needs.
- Emphasizes safety and alertness as a safety-sensitive position.
- Bachelor's Degree in Nursing
