Oak Street Health is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare in outpatient settings. The NP will work independently and with a collaborative team including physicians, NPs, PAs, medical assistants, scribes, and care managers to keep patients healthy and connected to their care plan.
Responsibilities include annual wellness visits, health risk assessments, care coordination, patient education, and participation in population health initiatives.
