Job SummaryPre‑licensure graduate of an accredited practical nursing program, possibly a participant in the SSM Health New Graduate Nurse Residency Program. Under the direction of an RN, provides care to patients within scope of role and professional standards, responsible for delivery of age appropriate care for assigned patients with a wide variety of medical conditions.
Responsibilities- Performs delegated tasks based on demonstrated competence in performing practical nursing care activities for the population served.
- Contributes to patient assessments including biophysical, psychosocial, spiritual, cultural, and age appropriate needs of the patients, including nutrition, pain management, and signs/symptoms of abuse or neglect to contribute to the patient's plan of care.
- Administers medications and observes patients for adverse reactions to medications and treatments.
- Educates patients/families about disease treatment plan including self‑care post discharge, holistic needs, available resources and follow up care.
- Completes accurate and timely documentation.
- Completes competency requirements satisfactorily and completes training as required; seeks assistance and asks questions as needed.
- Demonstrates professional nursing standards defined in the professional practice model;
Uses the ANA Code of Ethics for Nurses to guide professional decisions. - Works in a constant state of alertness and safety.
- Performs other duties as assigned.
- Graduate of an accredited school of practical nursing.
- For Illinois: an official passing score on NCLEX results.
- No experience required.
- Required professional license:
Licensed Practical Nurse (LPN) in the work location state, with passing NCLEX LP within 3 months of graduation. - Basic Life Support Health Care Provider (BLS HCP) – American Heart Association (AHA) mandatory for all states listed (Illinois, Missouri, Oklahoma, Wisconsin) with additional conditions for failed NCLEX within 3 months and requirement for LPN within 6 months or LPN Compact status.
- Illinois – BLS HCP and future status fail on NCLEX within 3 months.
- Oklahoma – BLS HCP, future fail on NCLEX within 3 months, future LPN within 6 months or LPN Compact State.
- Wisconsin – BLS HCP, future fail on NCLEX within 3 months, WI Nursing Temporary Permit.
- Constant use of speech and oral communication.
- Frequent standing, walking, frequent lifting/carrying/pushing/pulling objects 0‑25 lbs; occasional lifting up to 50 lbs; driving occasionally.
- Frequent sitting, reaching, keyboard use/data entry, vision and depth perception demands.
- Frequent use of smell, hearing and ability to detect alarms.
- Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
- Rare crawling and running.
