Location: St. Louis
SSM Health Cardinal Glennon Children's Hospital in St. Louis, MO, is seeking a part-time RN Nurse Navigator for its ACC clinic. The role supports patients across the continuum of care, coordinates with physicians and staff, and helps identify and address barriers to care.
Hours are Monday through Friday 8:00 am – 4:30 pm with two days per week scheduled. The navigator uses the nursing process to assess, plan, implement, and evaluate care, provides referrals for follow-up, and tracks outcomes to
