Responsibilities- Provide basic nursing care by assisting in the assessment of the resident on admission and on an ongoing basis.
- Remain knowledgeable about and communicate any significant change in condition to the appropriate interdisciplinary team (IDT) member.
- Accurately document all assessments and seek guidance if unsure of any assessment findings.
- Participate in the care planning process for the resident and update the care plan when specific interventions indicate the need for revision.
- Communicate the plan to the appropriate IDT members along with the residents and their families.
- Implement necessary and appropriate nursing interventions relevant to the plan of care, including nursing orders, physician orders, and residents’/families’ educational and environmental needs.
- Ensure interventions such as medication administration and treatments are accomplished in a safe and timely manner.
- Administer IV fluids only if IV certified.
- Document interventions accurately and timely.
- Use critical thinking when dealing with residents’ changes in condition.
- Notify the physician in a timely manner using the SBARR communication tool.
- Maintain composure during crisis situations such as family dissatisfaction, staff disputes, or inclement weather in a calm and professional manner.
- Monitor the care of residents to ensure the care plan is delivered effectively.
- Identify problem areas and use the QAPI process to actively address the situation for immediate problem resolution.
- Remain aware of residents’ status to quickly identify behavioral changes or other untoward signs or symptoms.
- Answer call lights and/or assist with residents’ ADLs as needed.
- Complete, at a minimum, twelve hours of nursing-related continuing education.
- Commit to staying current with best nursing practices through knowledge of Bethesda’s policies and self-study.
- May be assigned other duties as appropriate.
- Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse
- Long-term care/Rehab/Assisted Living experience preferred
- Current license in the state you will be working as a Licensed Practical Nurse
- IV Certification required or willing to obtain within six months of employment
- Current CPR/AED Certification required or willing to obtain within six months of employment
- Must be able to assess the basic nursing care needs of the geriatric resident
- Must possess excellent communication skills to interact with residents, visitors and staff
- Excellent time management and organizational skills are required
- Must possess critical thinking skills
$26.96 - $43.95 per hour
