Join Our 5-Star Team as an LPN/RN MDS Nurse Coordinator at Beauvais Rehab and Healthcare Center! We are currently seeking an experienced LPN / RN MDS Coordinator to join our leadership team. This role plays a pivotal part in maintaining the exceptional standards of care we are proud to provide.

- Collaborate with the Interdisciplinary Care Plan Team to develop resident‑specific care plans.
- Evaluate resident conditions and ensure all special assessments are completed as required.
- Provide data for the Facility’s Quality Assurance Program.
- Active and current LPN or RN license in the state of Missouri.
- Previous experience as an MDS Coordinator.
- Strong understanding of Medicare/Medicaid regulations and benefit guidelines.
