Role Overview

This position involves providing specialized nursing services in a home care setting, including patient assessments, developing and implementing individualized care plans, and coordinating care with an interprofessional team. The nurse evaluates patient needs, respects cultural and emotional considerations, and ensures quality, evidence-based care to promote patient safety and outcomes. Responsibilities include monitoring patient conditions, updating care plans, and advocating for patient-centered care.

• Assess patient preferences, barriers, and diverse needs
• Develop, document, and communicate personalized care plans
• Collaborate with team members and family/caregivers
• Evaluate patient condition changes and update care accordingly
• Ensure safe, timely, and evidence-based care delivery

• RN license with a nursing diploma/associate's degree (bachelor's preferred)
• Less than 2 years of experience; 2-5 years preferred
• Valid driver's license and ability to travel locally
• Strong communication, critical thinking, and cultural competency skills
• Ability to work in a safety-sensitive environment

• Part-time role (24 hours/week) with flexible day shifts
• Rotation required for weekends, holidays, and on-call duties
• Regular local travel within the service area, with mileage reimbursement in line with IRS guidelines
