Responsibilities include monitoring patient conditions, updating care plans, and advocating for patient-centered care.
Key Responsibilities- Assess patient preferences, barriers, and diverse needs
- Develop, document, and communicate personalized care plans
- Collaborate with team members and family/caregivers
- Evaluate patient condition changes and update care accordingly
- Ensure safe, timely, and evidence-based care delivery
- RN license with a nursing diploma/associate's degree (bachelor's preferred)
- Less than 2 years of experience; 2-5 years preferred
- Valid driver's license and ability to travel locally
- Strong communication, critical thinking, and cultural competency skills
- Ability to work in a safety-sensitive environment
- Part-time role (24 hours/week) with flexible day shifts
- Rotation required for weekends, holidays, and on-call duties
- Regular local travel within the service area, with mileage reimbursement in line with IRS guidelines
