RN Community Liaison

Mercy is seeking an RN Community Liaison to build strong relationships between our healthcare teams, patients, families, and community partners. In this role, you will coordinate transitions of care, provide education on services and resources, and promote Mercy's programs across the community. You'll collaborate with physicians, care managers, and external agencies to improve access, reduce readmissions, and support wellness initiatives.
