Overview

Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound and pain management services. The hospital has a 24‑hour adult emergency department and a separate pediatric emergency department in collaboration with St. Louis Children’s Hospital. The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliance Documentation Specialists and skilled administrative staff. Case Management in the hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The process encompasses communication and facilitates care along the continuum through effective resource coordination, balancing optimal health, access to care and appropriate utilization of resources with the patient’s right to self‑determination.

Selects, orients, monitors, assigns projects, counsels, problem‑solves, and leads employees under its jurisdiction. Practices in accordance with the Missouri Nurse Practice Act and HSO policies and standards of care. Leads and directs the coordination of patient care at the most appropriate location, level, and time.

• Manages individuals including onboarding and hiring, training, workload management, performance evaluations, and professional development plans to meet overall operational goals.
• Develops and adheres to departmental staffing and revenue/expense budgets, responding to changes that affect budget goals.
• Monitors, controls, and evaluates the resources used in Case Management, Social Services and Congestive Heart Failure Disease Management Departments.
• Directs the extension of care coordination in the inpatient setting.
• Provides designated service line physicians with timely data and assists in analysis to identify administrative focus areas.
• Oversees patients referred to the Hospitalist Service to ensure timely discharge and continuous communication with primary care physicians, patients, families, specialists, hospital departments and administration.
• Ensures safety‑sensitive performance, maintaining constant alertness and safe operation.
