Role Overview
Home Health RNs provide compassionate, skilled nursing care directly in patients' homes, coordinating interdisciplinary plans of care, assessing patient conditions, and supporting discharge planning. They perform tasks such as vital signs, wound care, IV and ostomy care, blood draws, and injections, ensuring accurate documentation and adherence to regulations. They collaborate with healthcare teams, communicate with patients and families, and respond to care needs and concerns.
Qualifications & SkillsGraduation from an accredited nursing program, valid RN license, and BLS certification. No prior experience required. Must have a valid driver’s license, insurance, and meet physical requirements including standing, walking, lifting, and driving. Strong communication, assessment, and documentation skills are essential.
Key ResponsibilitiesAssess patient needs, coordinate care, perform skilled nursing tasks, update physicians, document visits, and develop personalized care plans. Ensure safety, timely visits, and quality care delivery. Perform additional duties as assigned.
Additional RequirementsProof of insurance, valid driver’s license, and compliance with physical and licensure standards across multiple states. PRN schedule with flexibility for weekends and holidays. Benefits include paid parental leave, flexible pay options, and tuition coverage.
