Circle Health is seeking a licensed Nurse Practitioner (NP) to join our growing telehealth team in a fully remote teleconsulting capacity.

This role offers flexibility while contributing to high-quality virtual patient care services across chronic care and virtual healthcare programs.

• Active and unrestricted Nurse Practitioner (NP) license for |
  Missouri
• Must hold a valid license.

• Perform Initial visits.
• Conduct telephonic patient consultations
• Perform clinical assessments and develop treatment plans
• Support patient care coordination and follow-ups
• Maintain accurate clinical documentation
• Collaborate with internal clinical and operational teams as required

• Strong clinical assessment and patient management skills
• Comfortable working in a telehealth and remote care environment
• Excellent communication and patient interaction skills
• Ability to work independently and manage virtual consultations effectively
• Familiarity with EMR/EHR systems is preferred
