Role Overview:
Acts as a patient advocate by providing education, support, and interventions throughout the patient care continuum, including pre- and post-hospitalization. Collaborates with physicians to develop and implement treatment plans, monitors patient progress, and ensures smooth discharge transitions.
Responsibilities:Conducts new patient interviews, assesses health and educational needs, monitors lab values and test results, and provides patient/family education. Facilitates clinical protocols, assists with diagnosis, treatment, and rehabilitation processes, and serves as a resource for patients, families, and healthcare professionals.
Qualifications & Skills:Requires a Bachelor's Degree in Nursing and an active RN license. Preferred certifications include Oncology Certified Nurse (OCN) and Certified Heart Failure Nurse or Cardiac Nurse Specialist. Strong cardiology or ICU background is advantageous.
Additional Requirements:Ability to work in a safety-sensitive environment, maintain constant alertness, and collaborate effectively with medical teams. No supervisory experience required.
