Job Summary
The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during a patient’s hospital stay. Responsibilities include coordinating planning with other disciplines and adopting a patient/customer driven approach.
Job Responsibilities- Comprehensive Nursing Assessment:
- Perform age‑appropriate admission or transfer assessment.
- Obtain input from family/guardian when appropriate.
- Accurately document findings.
- Assess post‑op/post-invasive procedure patients.
- Document education and discharge needs at admission and throughout hospitalization.
- Provide reassessment and document observations per plan of care, changes in condition, and responses to care and procedures.
- Plan of Care Development:
- Establish, coordinate, and evaluate a plan of care based on assessment data, diagnoses, lab results, physician orders, protocols, and other relevant information.
- Identify short and long term goals, formulate nursing interventions, and incorporate disease‑specific evidence‑based practices into the care plan.
- Nursing Interventions:
- Collaborate with health team members for daily care plan coordination.
- Provide, coordinate, and communicate patient care, including handoff communication reports.
- Administer and document medications accurately according to policies and procedures.
- Monitor, maintain, and document IV fluids and parenteral nutrition.
- Complete referrals and request consultations for special needs, equipment, or information.
- Educate patient and family, plan discharge, clarify physician orders, and ensure timely implementation.
- Assist physicians with procedures or delegate to the Care Partner as appropriate.
- Document readback for telephone verbal orders and take orders only in emergency situations.
- Recognize changes in patient condition and take appropriate nursing actions.
- Involve family/guardian in care decisions when appropriate.
- Identify patient risks, act appropriately, and apply infection control practices.
- Nursing Care Review:
- Perform and document ongoing evaluation of care effectiveness.
- Evaluate effectiveness of patient/family education and modify the plan as needed.
- Report significant changes in clinical parameters immediately to physician and others as indicated.
- Identify problems, gather data, propose solutions, and communicate through appropriate authority lines.
- Report variations from care/treatment following occurrence reporting policy.
- Specialized Care (High‑Risk Patients):
- Restraint Care: Initiate/evaluate alternatives, apply restraints per approved procedure, monitor response, provide specified care, consult peers, and document thoroughly.
- Pain Management: Assess pain presence, incorporate cultural/spiritual beliefs, implement techniques, include family in plan development, and consider therapeutic methods.
- Abuse Assessment: Recognize abuse criteria, report signs to appropriate authorities, and take actions to support patient safety.
- Professional Practice:
- Adhere to quality standards, policies, and protocols; participate in learning experiences.
- Demonstrate appropriate technical and cognitive skills; keep up to date with unit information, policies, and procedures.
- Participate in committees, shared governance, staff meetings, and review hospital/nursing publications.
- Show ability to adapt to changing work demands and support peers.
- Orient, precept, and supervise new staff nurses; maintain collaborative team approach.
- Safety & Infection Control:
- Practice standard precautions and dispose of hazardous waste per guidelines.
- Maintain a safe, clean environment, follow hand hygiene, use safe patient handling equipment, and adhere to dress code.
- Report risk management concerns using eSRM.
- Mandatory Requirements:
- Maintain current RN license and certifications (BLS, HI, TB screening, influenza vaccination, ethics training, safety training, abuse & neglect course, blood‑borne pathogens training as required).
- Complete annual competency, population‑specific competency, and orientation competency as applicable.
