RN Case Manager

Christian Hospital is seeking a compassionate, organized, and collaborative RN Case Manager to join our team. The ED Case Manager plays a vital role in coordinating patient care, facilitating safe and timely discharges, reducing avoidable admissions, and connecting patients with appropriate post-acute resources.

• Previous case management or emergency department experience preferred
• Ability to work collaboratively in a fast paced environment

Northwest Health Care, six miles west of Christian, offers 24-hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services.

The Case Management Department provides services through a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's needs. The Case Management Department is responsible for all acute care inpatients where needs have been identified or who have requested assistance, emergency department patients where needs have been identified, and community individuals who request assistance.

Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.

Responsibilities

• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.
