• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in the state or ability to obtain it
• Reliable transportation and a valid driver’s license
• Ability to work in varied environments and perform physical duties as needed
• Preferred: 1+ years clinical experience in geriatrics, home health, or related fields; familiarity with EMRs and complex patient needs

• Conduct comprehensive in-home assessments and screenings
• Manage acute and chronic conditions, including testing and interventions
• Document medical information thoroughly and accurately
• Engage in care planning, counseling, and patient education
• Collaborate with the care team to ensure holistic patient support
