Description:
Provides coordinates and directs the provision of home nursing care according to physician orders based on agency policies and procedures.
Reports to Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits are required.
Periodic office meetings are included.
Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
Average number of patients per day is 5-8.Contractor will be assigned between 5-10 patients per day.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Weekend requirements vary per location and profession.
On-call requirements vary per location and profession.
Traveler is compensated from patient home to patient home.
The commute to and from the first and last patient are not billable.
Local candidates are accepted at the same rate.
Pending licenses are not accepted; must have license in hand.
All professions must be proficient with Home Care Home Base and have used it within the last 6 months.
Documentation must be completed within 24 hours of the appointment with the patient.
The clinician must accept every patient on their schedule regardless of distance.
No guaranteed hours are provided with this contract.
Clinicians must achieve weekly productivity points of 30, which equates to 40 hours per week.
Option to float to another clinic to pick up patients if there are not enough assigned patients.
Must be open to on-call rotations and traveling up to 75 miles from the assigned care center.
Local and traveler candidates qualify for the same rate.
All documentation impacting billing and follow-up patient care must be completed promptly.
Delays in documentation may result in cancelation.
